Kerala Lotteries Prize Claim
The agency will give a 10% discount on the prize money of 1 to 3 prize tickets to the agent who left the tickets.
4 to 8 Prize Tickets and Consolation Prize Tickets 10% of the prize money will be given by the agency as a gift from the government fund to the agent who sold the said tickets.
Tickets must be presented for prize distribution within 30 days after the draw. The first and second prize winners can collect the prize money either directly or through the Nationalized / Scheduled / State or District Co-operative Banks by presenting the required documents at the Lottery Directorate.
Self-attested copy of the gift ticket with the signature, name, and address of the prize giver, two passport size photographs attested by the Gazetted Officer, PAN card, photo affixed identity card with proof of address on the ticket, affidavit of the award giver's name, signature, and address.
The stamped receipt and a copy of the passbook with the recipient's bank account number (including IFS Code) should be produced with the ticket.
